WAIVER & RELEASE FORM

This waiver and release of liability includes, without limitation, all injuries which may occur as a result of; (a) your use of all amenities and equipment in the facility, Light Club sanctioned event space and technology, and your participation in any activity, class, program, facilitation or instruction, (b) the sudden and unforeseen malfunctioning of any equipment (c) Light Club LLC, all facilitator’s, training, supervision, or dietary recommendations and (d) your slipping and/or falling while in the facility, or on the facility premises, including adjacent sidewalks and parking areas.

You agree that if you engage in any physical exercise, relaxation practices or massage therapy on vibrational technology on the premises or off premises including any sponsored irevibe event, you do so voluntarily at your own risk. You agree that you are voluntarily participating in these activities, use of the Lucia Light, SoSound Yoga Boards and premises and assume all risks of injury, illness, or death. Light Club LLC, Charles Hyde, all owners and associates are not responsible for any loss of personal property or injury.

You acknowledge that you have carefully read this “waiver and release” and fully understand that it is a release of liability. You expressly agree to release and discharge Light Club LLC, Charles Hyde, and all affiliates, contractors, facilitators, owners, and partners from any and all claims or causes of injury and you agree to voluntarily give up or waive any right that you may otherwise have to bring a legal action against Light Club LLC events, Charles Hyde, owners and associates for personal injury or property damage.

To the extent that statute or case law does not prohibit releases for negligence, this release is also for negligence on the part of Charles Hyde, Light Club LLC, their agents, owners and employees.

If any portion of this release from liability shall be deemed by a Court of competent jurisdiction to be invalid, then the remainder of this release from liability shall remain in full force and effect and the offending provision or provisions severed here from.

At Light Club LLC, all events and experience are not specifically intended to cure, treat, prevent, or heal any disease, injury, or illness.  I understand that if I have a health problem, I will be referred to a licensed professional for further assistance upon my request or necessity.  

I understand the services provided by attendants and facilitators at Light Club LLC may cause certain  side effects such as:

  • fluttering of eyes

  • feelings of having transcended or left your body

  • visions

  • symbols

  • and/or emotional releases

  • in rare cases (including undiagnosed epilepsy) a seizure may occur.  

I understand that these effects may arise through no fault of Light Club LLC.

If I have any concerns about any of these possible side effects, I will keep light attendants at Light Club LLC fully advised about my concerns so appropriate intervention can be executed or revised to minimize discomfort. 

I understand that I am fully responsible for my own decisions and fully aware of the potential side effects. I will let the light attendant know if I feel uncomfortable at any point and the session will be ended.  


I understand that using the Lucia No. 3 light machine is not advised if I am:

  • Under 18 years of age

  • Pregnant

  • Having a health history of epilepsy or seizures

  • Having a health history of severe anxiety or other psychosis

  • Under the influence of any drugs or other mind-altering substances


I understand the above statements and it is my choice and consent to participate in this experience.  If I have any type of medical condition or if I am on any medication, I understand that it is my responsibility to consult my physician before engaging in this experience.  


By signing this release, I acknowledge that I understand its content and that this release cannot be modified orally.
